‘Skylar’

The PSCP is publishing its report into the Local Child Safeguarding Practice Review (LCSPR) of ‘Skylar’.

This case was considered by the Portsmouth Safeguarding Children Partnership (PSCP) at its Learning from Cases Committee in May 2021. The PSCP commissioned an independent reviewer to examine issues concerning the welfare of ‘Skylar’ and the lessons to be learnt to improve experiences for children in similar circumstances in the future, including the degree to which decisions were child focused and the effectiveness of working arrangements across agencies and services. Key themes within this report include:

  • Curiosity around fathers/partners
  • Escalating/receiving challenge
  • Using all known information to inform Assessments and decision making, historic and current. This includes health summaries and key information from other involved agencies
  • Identifying adult safeguarding concerns and referring as appropriate

The PSCP has also published a response document which outlines the actions that we have taken as a result of this report. All actions will be monitored on a quarterly basis by our Monitoring, Evaluation and Scrutiny Committee.
